TKA后关节线高度变化对膝关节力学特性影响

EFFECTS OF MAL-POSITION OF JOINT LINE ON KNEE BIOMECHANICS AFTER TOTAL KNEE ARTHROPLASTY

  • 摘要: 确定关节线高度变化与TKA后膝关节力学特性之间的关系能够有效的指导膝关节置换手术,从而更好的恢复膝关节的功能。该文通过对不同关节线高度下、两种不同的手术方案(髌骨保留和髌骨置换)的膝关节模型进行了有限元计算和分析。结果表明,关节线过高或者过低都会影响TKA后膝关节的力学环境,造成髌骨的运动失稳和聚乙烯部件的应力增加。在全膝关节置换的手术过程中,应尽量保持关节线的水平高度不要下移,关节线上移也不要超过4mm。该文的试验为全膝关节置换术评价及假体优化设计提供基本方法和数据。

     

    Abstract: A thorough investigation of the relationship between joint line and knee biomechanics after total knee arthroplasty (TKA) will guide the TKA surgery, and will improve the restoration of knee function. Different joint line models of two TKA surgeries (with patellar resurfacing and without patellar resurfacing) were developed and simulated in commercial finite element software. The simulation results show that if the joint line is too high or too low, the knee biomechanics will be affected, resulting in mal-motion of patella and high prosthesis stress. We draw the conclusion that the joint line cannot be moved down and the max displacement of moving up cannot be over 4mm. The simulation results provide basic method and data for assessing TKA surgery and prosthesis optimal design.
